- We
will then discuss with you the exact treatment or consultation
that you require, and confirm the price.
- We
will send you a confirmation of this together with a pre-surgery
enquiry form.
- Once
we have received the form we will again be in contact with you
to set a target date for hospital entry and assist you in making
your travel arrangements.
- We
will need your name, address and contact details.
- If
you are undertaking surgery we will also need the name, address
& contact details of your doctor or specialist and the completed
pre-surgery enquiry form.
- We
may need a copy of your medical records.
These
are of course kept confidential.
